pustrong$1000 Sign On Bonus, Starting Wage $15.25!/strong/u/p pspan style="font-family:inherit"Monday-Friday, agency vehicle provided for appointments./span/p pInterested applicants should be able to provide in a professional manner medical supports for individuals as directed by supervisor and/or a DSNWK nurse. nbsp;The advocacy and treatment of individuals served with dignity and respect is a prime responsibility./p pstrongEssential Functions/strong/p pParticipate and/or provide information in the life planning process for individuals as requested by Nursing Services./p pMaintain a cooperative and professional relationship among teams/peers by communicating information; responding to requests; building rapport; participating and contributing in team problem-solving methods. nbsp;/p pPromote individual#39;s independence by addressing medical treatments, assisting in understanding conditions, medications, self-care skills; answering questions as appropriate. Communicating these to Nursing Services for follow-up./p pProtect individuals and other employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations./p pPractice appropriate protective and preventative health measures for individuals served as per agency policy and other applicable regulations./p pPractice the safe and appropriate administration, storage, labeling and record keeping of medications. Ensure prescribed medications are available and ready to be administered./p pAssist in scheduling, providing transportation and attending medical appointments as outlined in each assigned individual#39;s life plan./p pEnsure Nursing Services is advised of doctor appointment results and responsible on-duty staff are made aware of medication needs./p pProvide teaching to individuals served on medical issues as directed by the agency nurse, and approved by supervisor./p pAttend initial and ongoing training in a timely manner to meet the needs of individuals served and meetings as requested by supervisor./p pMaintain documentation of individual#39;s care and treatments that complies with DSNWK policies, as well as federal and state regulations. nbsp;Complete all daily/monthly paperwork and documentation in a timely manner./p pRepresent agency with professionalism and advocate for individuals served at all times./p pParticipate in crisis intervention including physical restraint, lifting and transferring of individuals served, according to agency policy, as necessary./p pFamiliarize self with the rights of individuals served and ensure these rights are maintained at all times through appropriate verbal and physical interactions./p pFamiliarize self with and implement the risk management plan for each individual served./p pReport health, safety, licensing and abuse/neglect/exploitation (ANE) issues to appropriate personnel per agency policy./p pMonitor individuals health status and report changes to Nursing Services./p pEnsure individuals served are treated with dignity and respect to include using a respectful tone, giving full consideration and attention to each person served and providing opportunities and options that promote independence./p pPractice, monitor and ensure quality teaching of skills to individuals served to include but not be limited to recognizing opportunities to teach, providing rationales, offering positive reinforcement and encouraging decision making skills by each individual to promote independence./p pMaintain assigned cleanliness and Report any vehicle maintenance issues to secretary or transportation./p pObtain and remain current in providing Non-Emergency Medical Transportation (NEMT)./p pEnsure adequate amount of medical supplies are on hand, submit requisitions when they are needed./p pProvide assistance with Activities of Daily Living (ADLs) to include support and supervision during medical appointments and transportation etc./p pPromote community inclusion of individuals served./p pComplete other duties as assigned by the Nurse Manager and Director of Programs and Services./p pstrongHIPAA Compliance/strong/p pThis position is responsible for maintaining confidentiality for all PHI and ePHI that is shared with them. They will have access to all PHI and ePHI for individuals whom they support as a member of their Life Plan Team or for whom they provide back-up services or on-call supports./p pstrongOther Responsibilities/strong/p pMaintain technical knowledge by attending educational training as required by DSNWK and the State of Kansas./p pMaintains confidentiality and protects operations by keeping information confidential./p pAdministering Medication and Treatments, Verbal communication, Listening to understand, Infection Control, Medical Teamwork, Multi-tasking, Health Promotion and Maintenance./p pAccompany individuals served to hospital to assist with admission/discharge, when appropriate./p pImplement crisis intervention strategies including physical restraint, lifting and transferring of individuals served according to agency policy as necessary. nbsp;Attend Mandt Training./p pCoordinate and attend medical appointments and facilitate communication between individual(s), Nursing Services and the medical professional(s)./p pAbility to use computer and various programs for documentation./p pHearing and visual capabilities must allow individual to perform essential functions./p pDesire to work with adults with developmental disabilities./p pAble to exercise good judgment and discretion in handling confidential information./p pValid driver#39;s license./p pFirst Aid/CPR Certified.nbsp;strongWill Train./strong/p pThis position is subject to the two year training period as defined by KPERS, unless exceptions are granted due to prior employment.nbsp;/p pstrongJob Specifications/strong/p pstrongEducation: High School diploma or equivalent required.
